Navigate Health Lifestyle Program

Navigate Health is a lifestyle program, brought to you by Blackstone Valley Physical Therapy Services, Inc., that focuses on the overall health and wellness of the individual. Our goal is to help guide and support lifestyle changes that improve quality of life, decrease risk for chronic disease, and set you on the path to wellness.

HOW DOES IT WORK?

Navigate clients will partner with a health coach to discuss their wellness goals as they relate to any of the 5 foundational “elements” of health; movement, fuel (nutrition), recovery, endurance, and social connection. Our coaches take an individualized, client-centered approach that recognizes each persons’ unique qualities and goals. Acknowledging that change is hard, the Navigate program was built to assist clients in simplifying their goals, identifying manageable actions, and creating healthy habits that improve quality of life.

We are a team of physical therapists, health coaches, and personal trainers with a passion for helping others achieve a healthy lifestyle. We believe in a “whole health” approach and recognize that the foundation to optimal health lies not just in exercise and proper nutrition, but in stress management, sleep quality and quantity, and your level of quality engagement and connection with your community. We believe that fortifying our health, provides resilience in the face of inevitable stresses and challenges that life throws our way.

  • Health Assessment & Goals Questionnaire
  • Initial Goal Setting Meeting (Phone, Video, In-person discussion with your coach)
  • Weekly habit practice & check-in (15 min one-on-one discussion with your coach)

WHAT IS THE COST OF THE PROGRAM?

The cost of the program is less than $200/month and includes all scheduled calls/meetings with your coach. The initial investment in your health requires a 3-month commitment of $595 to initiate your habit-based, health coaching. We will begin with introduction call that will be 20-30 minutes long to establish your goals and the plan to achieve them. After the initial 3 months, monthly purchases are available to continue your progress.

CAN YOU GUARANTEE RESULTS?

Behavior change is a life-long commitment. Our coaches will work to assist you in achieving short-term, manageable goals. Your commitment to behavior change and your ability to communicate your needs and challenges with your coach will impact your success.

HOW DO I COMMUNICATE WITH MY COACH?

Sessions will be scheduled and take place over the phone, secure video chat, or in person based on your needs.

HOW DO I KNOW IF THIS PROGRAM IS RIGHT FOR ME?

The Navigate Health program is appropriate for anyone who is inspired to make a positive change in their health and wellness. If you are ready to make a change but don’t know where to start, this program is for you. If you have tried to make healthy changes before and felt overwhelmed, this program is for you. If you feel you’ve fallen off track with your healthy lifestyle and are having difficulty regaining momentum, this program is for you.

WILL YOU GIVE ME A SPECIFIC MEAL PLAN TO FOLLOW?

There is no specific meal plan or “diet” to follow. This actually works in your favor. We work with you to offer general advice on healthy eating, tips on making food prep easier, how to navigate the grocery store and make better choices at social events, etc. We believe these skills provide more sustainable long- term results than a specific short-term “diet”

WILL YOU TELL ME WHAT TO DO IN THE GYM?

Our program focuses on changing habits and offers general advice on how to get more movement into your life. We are able to provide online exercise programming and in-person personal training at an additional fee. Please ask your coach about details.

CAN’T I JUST DO THIS ON MY OWN?

Many people have success making lifestyle changes on their own, and we applaud them. These people are often motivated by internal accountability. More often, those who struggle to make long lasting change will benefit from the external accountability of a coach. Recognizing that you require some external accountability is not a weakness. You can turn this into a strength that helps you reach your goals when you find the right person or tool that helps to keep you on track.

IS THIS COVERED BY MY INSURANCE?

The cost of the program will be 100% the client responsibility. Currently most health insurances do not cover wellness and prevention services. You are always welcome to call your specific insurance plan and inquire about any potential subscriber reimbursement for wellness services.
